Job Description
VNS Health hospice services are provided by a team of experts who have the experience, training, and compassion to give your loved one the care they deserve. Care Manager
She previously worked in family medicine before transitioning to hospice and palliative care. Catalina Soto, DO, is a trained internal medicine physician. Dr. Soto thereafter undertook her internship training at Hackensack Meridian Health Palisades Medical Center in New Jersey, followed by her internal medicine residency at Orange Regional Medical Center in Middletown, New York. Dr. Soto is passionate about delivering high quality hospice and palliative medicine to members of the community.
Plans, organizes, and coordinates activities of the Clinical Team in assigned area. Ensures clinical area are compliant with Federal, and State, regulations, as well as agency policies and procedures and holds staff accountable. Ensures quality patient care delivered by clinical staff utilizing objective outcome based criteria. Hospice care also supports you and other caregivers and family members during your loved one’s illness and beyond.
REHABILITATION THERAPY
To be eligible, a participant has his or her doctor send a completed Physician´s Order to the local social services district, which then arranges a social and nursing assessment of the individual. A nurse assessor uses the results of the assessments and the physician’s order to recommend an appropriate amount, frequency and duration of services.
